Management Plans

At enrolment, you will need to provide OCK a management plan if your child suffers from:

• asthma,

• anaphylaxis or

• any other allergies

• regular medication plans

This information enables our educators to follow the appropriate steps to support your child while at kindergarten.

Make an appointment with your doctor to complete the right plan. If you require a different management plan to these provided above, please visit the ASCIA website.

We also ask you complete an individual anaphylaxis care plan or an individual asthma minimisation plan in addition to the management plan.

Your child cannot attend OCK without an appropriate plans, medication or epipen, Please speak to our teachers if you have an questions.

Inclusion Support Plan

• If modifications are required to ensure your child can fully engage at OCK you will need to complete an Inclusions Support Plan
